Who is eligible to participate in the Wellness Program?
Employees and spouses covered on a CHI medical plan are eligible to participate in the Wellness Program.

Is my spouse eligible to participate in the Wellness Program?
Your spouse — if enrolled in a CHI medical plan — has access to the Wellness Program and incentives, too. To send your spouse an invitation to participate in the Wellness Program, see the “Spouse Participation” section of this guide.

Are dependents eligible to participate in the Wellness Program?
Dependents, including legally domiciled adults and children, are not eligible to participate in the Wellness Program or earn wellness dollars.

When am I eligible to participate in the Wellness Program?
You are eligible to participate in the Wellness Program the same day that your benefits are effective. Please contact HR/Payroll Connection Support Center at 1-844-450-9450 with any benefits related questions.

Why am I having trouble accessing the wellness website through my internet browser?
Your wellness website works best on iPhone or Android devices and in Internet Explorer 11 or Google Chrome web browsers. If you currently do not have the recommended browser listed above, please contact your local IT department (not Preventure) to assist you with the set-up and support of Firefox as an alternative browser.

Do I have to spend my Health Account dollars by 10/31/2018?
No, only gift card incentives must be redeemed by 10/31/2018. In accordance with IRS regulations, all Health Account contributions deposited into an HSA are the employees. Dollars deposited into a Health Incentive Account are available for use as long as the employee is employed and covered on a CHI medical plan.

How will I receive my gift card?
All gift cards are delivered electronically via email. Some brands offer only online redemption  (Amazon.com for example) while others allow in-store redemption. The redemption instructions reference both options when available.

Do gift cards redeemed for participation in the wellness program expire?
Only Visa gift cards have an expiration date. Gift cards in the US are not allowed to expire. The Visa cards are considered a “Cash Equivalent Card” and thus have an expiration date.

How will I be taxed for gift cards?
You will be taxed in December for all gift cards redeemed during the program year. Please note, you will be taxed for all gift cards redeemed by your participating spouse too.

Do wellness dollars contributed to a Health Savings Account count toward the IRS maximum?
Yes, wellness dollars contributed to a HSA by CHI count toward the IRS maximum. (For 2018, the IRS maximum is $3,450 for single coverage and $6,900 for other coverage levels.)

Where can I go to see all of the programs I can participate in to earn wellness dollars?
From the wellness website, go to Menu and select Earn. The Earn page is where you can go to see all of the programs available to earn rewards. You can also track your progress here.

Where can I go to see how many wellness dollars I’ve earned?
From the wellness website, go to Menu and select Earn. The Earn page is where you can go to see all of the programs available to earn rewards.

Is my work location going to offer onsite biometric screenings?
Although this depends on your specific location, the majority of CHI locations will not offer onsite biometric screenings like we’ve done in the past. You can still earn a $50 incentive for a completing a biometric screening through your provider. Preventive and Biometric Screening Forms must be completed and signed by your provider. Completed forms must be submitted following the process outlined on the wellness website. Please visit the Preventive Health Screening page on the wellness website for more information.

Can I still earn dollars for outcomes from a biometric screening?
No, participants can no longer earn outcomes dollars from biometric screenings. You can still earn a $50 incentive for a completing a biometric screening through your provider. Preventive and Biometric Screening Form must be completed and signed by your provider. Completed forms must be submitted following the process outlined on the wellness website and in this guide. How do I submit Preventive Health and Biometric Screening Form(s)?
Completed forms can be uploaded via the wellness website. If you are experiencing issues or are unable to upload your form using a computer, please contact Preventure at 888-321-4326.

Can I submit multiple preventive exams on the same form?
A separate Preventive and Biometric Screening Form must be submitted for each individual exam you complete.

Where do I go to see my biometric screening results from last year?
From your dashboard, go to the menu and select “Act”. Scroll down to the Biometrics section and select “Biometric Screening Results”. Select the date you want to view from the drop down at the top of the page to review results from previous screenings.

Where do I go to see my Health Assessment results from last year?
From your dashboard, go to the menu and select “Act”. Scroll down to the Healthy Assessments section and select “Health Assessment Reports”. Select the date you want to view from the drop down at the top of the page to review reports from previous screenings.

How do I find out who my wellness champion is?
Visit the CHI well-being pages (formerly known as My Healthy Spirit) on InsideCHI, select Wellness from the top navigation bar and scroll down to the “Your Wellness Champion” section. Your wellness champion’s contact information can be found here.
