Your Tax Forms: W-2 and 1095-C
Tax season is approaching so we want to remind you what tax forms you should expect to receive and how we will get them to you. It is important to make sure you know why you receive the forms, when you should receive them and what your responsibilities are for filing them with the Internal Revenue Service (IRS).
What forms should I expect to receive?
CHI will provide you with tax forms W-2 and 1095-C. These forms contain information you may need to report as part of your individual income tax filing.
What is form W-2 and why do I receive it?
Most of us are familiar with form W-2, it is proof of the wages you earn each year. The W-2 provides details of your income, taxes paid and pretax contributions made for your benefits. You will need to transfer the information reported on form W-2 to your individual income tax filing.
What is form 1095-C and why is it important?
The IRS requires us to issue an IRS Form 1095-C to all employees who were enrolled in or offered employer-provided health care coverage at any time during the previous tax year. Think of the Form 1095-C as your proof of insurance for the IRS. Your 1095-C contains information about you and any family member who may have enrolled in your health insurance coverage offered during the previous tax year.
When preparing your individual tax return you may need form 1095-C to demonstrate to the IRS that you did or did not enroll in health insurance coverage to ensure that you and CHI are in compliance with the Affordable Care Act’s health insurance coverage mandates. Just like form W-2, simply transfer the information on form 1095-C to your individual tax return filing.
It is important to note that you may receive more than one 1095-C (and/or a Form 1095-B, if coverage was offered through an insurance carrier) if you received insurance coverage from another company last year in addition to CHI.
When will I receive my W-2 & form 1095-C?
If you have previously provided consent to receive the W-2 and 1095-C tax forms online, you will have access to view and print them from HR/Payroll Connection. Your W2 form was made available on January 8, 2020. Your 1095-C will be available no later than February 9, 2020. For convenient access to the forms, you can log into HR/Payroll Connection from work or home.
Once you are logged in, simply follow these steps to view your tax forms:
- Click the “View My Pay” link in the middle of the screen
- Click the “Access Current Year-End Statements” under Year-End Tax Statements section
- Place check mark on 2019 W-2 and 2019 1095-C
- Select “DOWNLOAD”
- Click “Review & Complete Order”
If you did not provide consent to receive your forms online on or before January 5, 2020, your 2019 forms W-2 and 1095-C will be mailed to your mailing address as it is identified on the My Information tab of HR/Payroll Connection. Your tax forms will be postmarked by January 31, 2020. Both your W-2 and 1095-C forms will then be made available online beginning February 9, 2020 to provide you with access to reprint a form in case of loss.
