Earn $50 Per Preventive Screening in 2019

Want to live a long and healthy life? Then make preventive care part of your health care habits and earn $50 in wellness dollars per exam. Earning wellness dollars for each preventive exam is easy. Simply schedule your exam and be sure to take your Preventive Health Screening form with you to the doctor’s office, forms are located on the “Programs” page on your wellness website. If you have already had your preventive screening you may submit a copy of your screening/exam results in lieu of a physician’s signature or you may ask your provider to complete and sign the form now. Submit a form for each exam – up to a maximum of four submissions per program year.

Preventive Screening Form Process:

  • Complete form in full. One exam per form submission.
  • One reward opportunity per unique exam up to a max of four submissions per year
  • Screenings that occurred between September 29, 2018 – September 30, 2019 are valid
  • Can submit screening results along with form in place of provider’s signature
  • Submit one of the following ways:

Preventive Screening Forms can be submitted between January 14, 2019 – September 30, 2019 for screenings or exams that occurred between September 29, 2018 – September 30, 2019. You can expect to see your wellness dollars approximately 10 days after form submission.
