Tonja Jones, RN was caring for a patient in the emergency department who had an order written to administer Heparin. Tonja reviewed her orders and lab values for the patient.  Upon careful examination she found the patient to have abnormally high coagulation levels.  She immediately notified the physician of the patient’s coagulation lab results.  The Heparin was promptly discontinued.

When Tonja received an order for her patient she Had a Questioning Attitude and investigated further.  She used the SafetyFirst technique Stop and Resolve.   She prevented her patient from receiving an unnecessary dose of the blood thinner Heparin.  This could have been dangerous to the patient.
