Transition of Care Clinic Team Wins Team Service Hero Award
Congratulations to the amazing team at the CHI St. Vincent Hot Springs Transition of Care Clinic for winning the team Service Hero award!
This team, including Dr. Richard Chastain, Cheryl McClard and Simone Gayle, were nominated for the award by a coworker.
The Transition of Care Clinic was opened in May 2018 and provides a bridge for care after the patient is discharged from the hospital or emergency department, but before they are fully recovered and on the road to their best health.
“For those of you who are not aware, the Transition of Care Clinic plays a critical part in caring for the patients in our community.”
The coworker said Simone and Cheryl keep the clinic running smoothly. Simone in her role of the clinic nurse is there by Dr. Chastain’s side every step of the way, ensuring patients get timely and appropriate care. Cheryl assists on the front end, handling the enormous role of getting patients into the clinic within 7 days of discharge from the hospital. According to the coworker, Cheryl also schedules follow up patients from the ED and Dr. Chastain’s own clinic patients.
“When Cheryl was out for medical leave a few weeks ago we truly felt the significance of the impact of her behind the scenes work,” said the coworker.
The coworker also raved about Dr. Chastain. They said he has embraced his role as physician leader of the clinic and works with the medical residents rotating through his clinic.
“His ministry is making a difference for countless difficult to treat and care for patients. He is passionate about patient care and about ensuring all patients receive access and excellent care.”
According to the coworker, Dr. Chastain really shines when working with difficult and high risk patients. They said he understands the complexities of navigating throughout the healthcare maze and persists with developing the best plan for each individual patient to get them back to their baseline and keep them out of the hospital.
“As the ‘captain of the ship,’ he is a great communicator, has improved transparency between multiple physician disciplines, thinks outside the box, and collaborates with home health, care coordination, physicians, and others – always with the patient and excellent care in mind.”
It’s that excellent care that led the coworker to nominating this team for the award.
“It is truly a privilege to work with them all – they keep the bar elevated and expect the same from others.”
