Prizes will also be drawn if campuses reach their participation goal.

For North, Morrilton and Heart Institute:

• If any of the campuses reach 90% participation by May 7, survey participants at that campus will be entered into a drawing for a $750 gift card. One gift card will be awarded at each campus.
• If any of the campuses reach 100% participation by May 7, survey participants at that campus will be entered into a drawing for a $1,000 gift card. One gift card will be awarded at each campus.

For Infirmary, Hot Springs and Medical Group:

• If any of the campuses reach 85% participation by May 7, survey participants at that campus will be entered into a drawing for a $750 gift card. One gift card will be awarded at each campus.
• If any of the campuses reach 90% participation by May 7, survey participants at that campus will be entered into a drawing for a $750 gift card. One gift card will be awarded at each campus.

Surveys must be completed by May 7.

All survey responses will be anonymous and confidential. Results will be shared later this summer.
