Enter to Win Prizes When You Participate in Coworker Survey (PCA)

The Coworker Survey (PCA) begins Monday, April 16, and coworkers who complete the survey the will be entered to win amazing prizes!

Daily prizes will be awarded the first week. To be entered for a daily prize, visit one of the rooms to take the survey and submit your ticket into the drawing. If you take the survey at your desk, fill out a green ticket provided by your leader; clinics will email their clinic leader.

Prizes will be drawn daily throughout the first week. If you turn your ticket in the first day, it will stay in the drawing unless you win a daily prize. Daily prizes include:

  • Day 1: 4 Magic Springs Season Passes
  • Day 2: Apple Watch
  • Day 3: Samsung Galaxy Tablet
  • Day 4: Yamaha Home Theater Speaker System
  • Day 5: Bose Soundlink Bluetooth Speaker
  • Day 6: Amazon Echo, Second Generation

Grand Prizes:
If your Mission Matchup team wins their contest, your name will go in the drawing for a grand prize of a 49″ Smart TV. There will be three winners; one from each bracket (Hot Springs vs Infirmary; Morrilton & Quality vs. North & Arkansas Health Network; and Medical Group vs. Heart & System).

Departments that reach 100% participation will have an additional entry into the drawing. All winners of the daily prizes are also eligible to win a grand prize.

Department Participation Drawings:

  • Departments with 100% participation on the first day will be entered into a drawing for the Traveler’s Box or a Game Day Package (depending on the size of the group)
  • Departments with 100% by April 29 will also be entered into a drawing for the Traveler’s Box or a Game Day Package (depending on the size of the group)
  • In addition, any department with 100% will receive a departmental meal or treat from Sodexo

For North, Morrilton and Heart Institute:

  • If any of the campuses reach 90% participation by May 7, survey participants at that campus will be entered into a drawing for a $750 gift card. One gift card will be awarded at each campus.
  • If any of the campuses reach 100% participation by May 7, survey participants at that campus will be entered into a drawing for a $1,000 gift card. One gift card will be awarded at each campus.

For Infirmary, Hot Springs and Medical Group:

  • If any of the campuses reach 85% participation by May 7, survey participants at that campus will be entered into a drawing for a $750 gift card. One gift card will be awarded at each campus.
  • If any of the campuses reach 90% participation by May 7, survey participants at that campus will be entered into a drawing for a $750 gift card. One gift card will be awarded at each campus.
